Electronic Submission of OSHA 300A Data – Overview

Establishments with 250 or more employees, or with 20 – 249 employees in certain industries, are required to submit OSHA 300A information electronically. To be compliant with OSHA’s federally required standards, Coins ERP+ has an OSHA Form 300A Export function that allows you to create a comma-separated file (.csv) for electronic submission.

This topic describes the functionality that allows the export of OSHA 300A information.

Configuration

The Health & Safety OSHADIR parameter allows you to define the directory in which the OSHA exports should be saved.

The OSHA Exports menu with an OSHA Form 300A Export function is found on the Health & Safety Reports menu, as shown in H&S OSHA Exports Menu.

Selection Screen

When you select the OSHA Form 300A Export option, a selection screen opens (see OSHA Form 300A Report Selection Screen), which you can use to export the OSHA 300A forms.

Field Definitions (Selected Fields)

Companies

You can select one or more companies, which help determine the jobs to be used.

[Accident Analysis Sets]

Accident Analysis Sets found on this screen (such as Litigation Status, Injury Type, Disability) are set up in H&S Configuration, as shown in H&S Configuration. They are not company-specific.

H&S Configuration

Output File Name

The OSHA Output File Name defaults in as “OSHA300A” plus the current year (such as OSHA300A_2019.cxv), but it can be renamed.

Report Sample

The OSHA 300A Export report only shows the fields required by the exported records. A sample of this report that includes some of these fields is shown in Sample OSHA 300A Summary Export Report.

Report Field Descriptions (Selected Fields)

Descriptions of fields that may appear on the OSHA 300A summary export report are listed below. Note: This is not a complete list.

Field

Description

Establishment

The unique name of the establishment reporting data.

Address

The street address of the establishment.

City

The city where the establishment is located.

State

The 2-character postal code for the U.S. State or Territory in which the establishment is located.

Zip

The full zip code for the establishment.

NAICS Code

The North American Industry Classification System (NAICS) code that classifies an establishment’s business. The value comes from System > Company Setup > Configuration > CO Configuration.

Size

The size of the establishment based on the maximum number of employees who worked there at any point in the year in which data are being submitted.

  • 1 equals < 20 employees

  • 2 equals 20-249 employees

  • 3 equals 250+ employees

Year

The calendar year in which the reported injuries and illnesses occurred at the establishment.

Average Number of Employees

The annual average number of employees. This number is between 1 and 24,999.

Total Hours Worked

The total hours worked last year divided by all employees. This number is greater than 500 and less than 8760.

Days Away From Work

Total number of cases with days away from work.

Days Restricted

Total number of days of job transfer or restriction (Form 300A Field I).

Total Injuries

Total number of injuries (Form 300A Field M(1)).

Total Skin Disorders

Total number of skin disorders (Form 300A Field M(2)).

Total Respiratory Conditions

Total number of respiratory conditions (Form 300A Field M(3)).

Total Poisonings

Total number of poisonings (Form 300A Field M(4)).

Total Hearing Loss

Total number of hearing loss (Form 300A Field M(5)).

Total Other Illnesses

Total number of all other illnesses (Form 300A Field M(6)).

Total Deaths

Total number of deaths (Form 300A Field G).
